Use of the Prescriber Portal by Health Care Providers

The Prescriber Portal is for use by health care providers only.  Patients and health care consumers are restricted from accessing the Prescriber Portal.

If you are a health care provider, when you provide information about yourself to us, including when you create an Account, you agree to: (a) provide accurate, current, and complete information about yourself and/or your facility, and not to provide information that attempts to impersonate another individual or facility; and (b) maintain and promptly update such information to keep it accurate, current, and complete.

Health care providers represent and warrant that when using the Prescriber Portal you will comply with all applicable laws and regulations, including the Health Insurance Portability and Accountability Act (HIPAA) and other applicable federal and state privacy and data security laws.

The Prescriber Portal requires you (health care providers) to register and obtain a password prior to accessing the portal. You acknowledge and agree that you are responsible for maintaining the confidentiality of your login ID and password.  You agree to notify us immediately of any unauthorized use of your login ID, password, or Account.  You acknowledge that you may be held liable for any loss or harm incurred by us or any other person or entity due to unauthorized access of your Account as a result of your failing to keep your Account information secure and confidential.

Operation of Online Services

We reserve the right to do any of the following at any time, at our sole discretion, with or without notice: (i) modify, suspend, or terminate operation of your access to our Online Services, or any portion of our Online Services, including but not limited to your violation of these Terms and Conditions; (ii) modify or change our Online Services, or any portion of our Online Services; and (iii) interrupt the regular operation of our Online Services, or any portion of our Online Services, as necessary to perform maintenance, to correct errors, or to make other changes to our Online Services.
